The New Jewish Home is one of the nation’s largest and most diversified nonprofit older adult health care systems, providing rehabilitation, skilled nursing and health care programs, which serves 12,000 older adults of all faiths and ethnicities annually.

The Adult Day Health Care Program provides a diverse array of physical and mental health services for adults who live with chronic health conditions including, for example, heart disease, diabetes, behavioral health problems, and Alzheimer’s and dementia, as well as for the very frail, the cognitively impaired and those who need rehabilitation.

Among the available services are:  physical, occupational and speech therapy; a behavioral health program; diabetes management; medication management; health monitoring; skilled nursing care; nutritional counseling; and social work services. A special night care program provides a safe place for people with Alzheimer’s disease and other dementias, giving family members and caregivers much-needed respite and relief.

The services are provided by a supportive, nurturing and highly-skilled staff comprising a medical doctor, nurses, a certified psychiatric nurse, speech, occupational and physical therapists, a nutritionist, an alcohol and substance abuse counselor, and social workers.

Jeffrey Farber, M.D., President and CEO of The New Jewish Home, said, “For more than a century, The New Jewish Home has been providing older New Yorkers with high quality and compassionate health care. With the opening of our new state-of-the-art Adult Day Health Care Program, we are bringing critically-needed support services and high-quality person-directed care to the community – all in a beautiful, safe and caring environment.”
