The Adult Day Health Care Program provides a diverse array of physical and mental health services for adults who live with chronic health conditions including, for example, heart disease, diabetes, behavioral health problems, and Alzheimer’s and dementia, as well as for the very frail, the cognitively impaired and those who need rehabilitation.
Among the available services are: physical, occupational and speech therapy; a behavioral health program; diabetes management; medication management; health monitoring; skilled nursing care; nutritional counseling; and social work services. The services are provided by a supportive, nurturing and highly-skilled staff comprising a medical doctor, nurses, a certified psychiatric nurse, speech, occupational and physical therapists, a nutritionist, an alcohol and substance abuse counselor, and social workers.
